NAP Admission Checklist
Submit to Millikin University School of Nursing, 1184 West Main Street, Decatur, Illinois 62522 (copies will be provided to DMH by MU):
- Complete the MSN application https://muonline.millikin.edu/prod/bwskalog.p_disploginnew
- Send official transcripts from all graduate and undergraduate institutions attended. If your undergraduate cumulative GPA is below 3.0, provide scores on the Graduate Record Exam (GRE).
- Three letters of recommendation (One must be from a direct ICU supervisor)
- a one to two-page written statement (up to 1000 words) addressing personal and professional goals related to the master’s program via either email to mgtaylor@mail.millikin.edu or mail
- a current resume or curriculum vita via either email to mgtaylor@mail.millikin.edu or mail
Submit to the attention of Nurse Anesthesia Program at Decatur Memorial Hospital, 2300 N. Edward St., Decatur, Illinois 62526 (copies will be provided to MU by DMH):
- a copy of your current Registered Nursing licensure.
- Completed Assessment of Critical Care Skills form
- Current verification of successful completion of Basic Cardiac Life Support (BLS); Advanced Cardiac Life Support (ACLS); and Pediatric Advanced Life Support (PALS).
